Families supporting a neurodivergent child or an adult with a disability often manage a wide range of services, appointments, funding renewals, documentation, and communication between professionals. Over time, coordinating these moving parts can place significant pressure on caregivers and reduce overall family capacity.
Family Systems Coordination Support is designed to reduce administrative overwhelm by helping families organize and maintain the systems connected to care and support services. This program offers structured coordination assistance so families can better track responsibilities, maintain continuity of services, and connect with appropriate community-based supports. This service is administrative and coordination-based in nature and does not replace clinical, therapeutic, medical, or legal services.
What’s Included
Systems Mapping & Coordination Review
The program begins with an initial 60 minute consultation, offered virtually or in-home, to review the family’s current care system. This may include service providers, therapy schedules, medical appointments, funding sources, and documentation requirements. The goal of this session is to understand the family’s current systems and identify practical opportunities to reduce coordination barriers and caregiver stress.
Ongoing Coordination Support
Families receive up to 3 hours per month of structured coordination support. This may include calendar organization, appointment tracking, scheduling or rescheduling when requested, monitoring funding timelines, organizing records, communicating with service providers with written consent, and helping families connect with appropriate community-based services. Support may include both direct communication and behind-the-scenes administrative coordination.
Systems Stability & Future Planning
As systems are established, ongoing review helps ensure they remain manageable and sustainable over time. This part of the program focuses on helping families maintain organization and identify practical strategies that support long-term success in managing disability-related supports.
